Admission Procedures

Patients should register at Hospital's Admission Office (Address: Block L, Ground Floor of Main Block). Admission slip (for scheduled admission) and the following documents are required :

Patient Status Identification Documents
Holder of HKID Card HKID Card/ Passport
Child (under 11 years of age) Hong Kong Birth Certificate;
HKID Card from father/ mother
Spouse of a HKID Card Holder Marriage Certificate and
HKID Card from Spouse
Valid travelling document (if necessary)
Child (under 11 years of age) of a HKID Card Holder Birth Certificate with father/ mother's name and
HKID Card from father or mother
Valid travelling document (if necessary)

Patients’ Charter

Hospitalization

  • Do not bring large amount of cash, jewelry etc. during hospitalization.
  • Do not leave the ward without notifying ward staff or the hospital will assume no responsibility for any consequences.
  • Patient's next-of-kin could arrange a meeting with the doctor through the attending nurse to know the latest medical condition of the patient.
  • Turn off mobile phones and other electronic devices inside the wards and designated areas to avoid interfereing the operation of medical equipment.
  • Smoking is strictly prohibited in the hospital compound.
  • No photography and videography by mobile phones, camera or other devices are allowed inside the wards.
  • Offering advantage to hospital staff is strictly forbidden.
  • Children under 12 are not allowed to go inside the ward.

Discharge

Transfer Procedures

Hospital Authority provides services through hospital clusters. Patients will be transferred from general hospitals to convalescent hospitals for continuous treatment in accordance with their medical condition.

Discharge Procedures

Ward staff will assist with discharge arrangement. Patients having financial difficulty may contact ward staff or medical social worker to apply for waiver of hospital fees.

Your valuable suggestion or feedback regarding the services provided by the Hospital can help us improve our quality. Please give your suggestion or opinion to Patient Relations Officer or Ward Manager.

Patient Relations Officers

Patient Relations Officers are appointed in each hospital. Their duties include answering enquiries, receiving feedback from patients and their relatives on the services of the hospital. They also ensure that complaints on services of the hospital are handled promptly and appropriately.
