Collection of Death Documents
After receiving the call from our staff, please come to the Death Documentation Office during office hours to collect the death documents.

Please present the following identity documents to the Death Documentation Office:
- Original Hong Kong identity document of the deceased
- Original Hong Kong identity document of the relative who will collect the deceased’s body
*For normal death cases, Death Documentation Office will typically issue the following documents:
- Medical Certificate of the Cause of Death
- Medical Certificate (Cremation) - if the body is to be cremated
- Body Collection Form

Collection of Body
Upon the patient’s passing, the body will be transferred to ‘The Haven’ at LG4 of Block K for temporary storage before being transferred to the ‘Bereavement Suite and Mortuary’ on the next day.

The hours for body collection are between 9:00am – 12:45pm and 2:00pm – 4:45pm every day. Body collection is based on a 'first-come, first-served' principle, and each session lasts 15 minutes. Late comers are required to wait in the waiting area.
According to the Hospital Authority Body Collection Policy, relatives or authorized representatives must bring the following original documents to the mortuary for body collection. The body will NOT be released if the required original documents are not presented.
For Normal Death Cases:
- Original "Body Collection Form"
- Original "Certificate of Registration of Death" (Form 12)
- Original Hong Kong Identity Document of the deceased
- Original Hong Kong Identity Document of the relative or authorized representative
For Coroner’s Court Cases:
- Original “Body Collection Form”
- Original “Certificate of Order Authorizing Burial / Cremation of Body” (Form 11)
- Original Hong Kong Identity Document of the deceased
- Original Hong Kong Identity Document of the relative or authorized representative
Relatives will be invited to identify the body and confirm the identification information of the wristband attached to the body. Relatives and friends may accompany the viewing of the body to confirm the identity of the deceased.
After the completion of body identification procedure, the body will be released to the licensed funeral parlour who are appointed by relative.
If the person collecting the body is not the one indicated on the “Body Collection Form”, authorization must be presented by completing the authorization part of the “Body Collection Form.
Loss of Original Documents
In the event of losing any relevant original documents, the relative must report to the nearby Police Station as soon as possible. The relative or authorized representative must present the “MEMO of the Report of Lost Property” issued by the Police Station during body collection. If the lost documents include the “Body Collection Form”, “Medical Certificate (Cremation)” (Form 2), “Certificate of Order Authorizing Burial / Cremation of Body” (Form 11) and/or “Medical Certificate of the Cause of Death” (Form 18), after reporting the lost document(s) to the Police, the relative must contact the Death Documentation Office for re-issuing such document(s) as soon as possible during office hours.
Viewing Body
Relative can view the body in the Bereavement Suite and Mortuary one day after death. Please present the identity document of the deceased during office hours to apply for viewing. Each viewing session is limited to 15 minutes. For appointments, please call the Bereavement Suite and Mortuary during office hours.

Farewell Room
The Farewell Room is located on the 1/F, Block T of Queen Mary Hospital. It is free-of-charge and operates on a first-come, first-served basis, with each session lasting 1 hour. Relatives can hold a simple funeral ceremony in this room during office hours before the body is transferred out.

Organ Donation
For organ donation inquiries, relatives can contact ward staff. Relatives who wish to donate organs after death can complete the registration form in the organ donation promotional leaflet or register online via the website below.

Additional Information
The body storage capacity of Bereavement Suite and Mortuary is limited. Prolonged storage of bodies in refrigerator with low temperature may adversely affect the appearance of the deceased. It is strongly advised to collect the body as soon as possible.
Possible changes to the body after death:
- Leakage of body fluid and skin peeling
- Body dehydration
- Muscle stiffening and softening due to decomposition
- Body colour change
- Infection
Relatives are advised not to leave any valuables with the deceased, as the hospital will not be responsible for any loss or damage.
Hospital mortuary services are free-of-charge. All hospital employees (including mortuary staff) are prohibited from receiving any money or gifts. They are also not allowed to provide advice on funeral services.
