Clinical Pharmacists provide pharmaceutical care to patients

The Department of Pharmacy of Queen Mary Hospital (QMH) provides quality, patient-centred and holistic pharmaceutical care to the public in Hong Kong. Queen Mary Hospital is a teaching hospital founded in 1937 with around 1,700 beds. Affiliated with the University of Hong Kong, the Department of Pharmacy provides professional training and education to medical, nursing and pharmacy students.

The Department of Pharmacy provides 24-hour-a-day, 7-day-a-week pharmaceutical services to all inpatients and outpatients, including patients from Specialist Outpatient Clinics and Accident & Emergency Department. The Department of Pharmacy is providing pharmaceutical care to approximately 30,000 outpatients and 8,000 inpatients per month. In addition to medication management, the Department of Pharmacy is clinically oriented which provides a great variety of Clinical Pharmacy services. Queen Mary Hospital was also accredited with an "Excellence" Achievement on medication safety by the Australian Council on Healthcare Standards (ACHS) in 2010.

The Department of Pharmacy of Queen Mary Hospital is the pioneering site for many Hospital Authority’s initiatives including: Clinical Pharmacy Services, Pharmacist-led Ambulatory Clinics, Medication Delivery Service and Telepharmacy Service, Clinical Trials Service, Medication Incident Reporting Programme, Drug Information Kiosk and 24-hour pharmaceutical services.

General Service
  • Provide 24-hour pharmacy services to inpatients and outpatients including patients from Specialist Outpatient Clinics and Accident & Emergency Department
Clinical Pharmacy Services
  • Services cover general Paediatric wards, Paediatric Intensive Care Unit (PICU), Neonatal Intensive Care Unit (NICU), medical and surgical admission wards, Haematology and Oncology wards
  • In 2022, the Clinical Pharmacy Services have been extended to Adult Intensive Care Unit (AICU) which provides pharmaceutical care to critical adult patients including those with severe liver failure waiting for liver transplants in Hong Kong
  • Attend ward rounds with physicians and other healthcare professionals. Formulate drug treatment plan and monitor the therapeutic outcomes of patients
  • Provide medication review and medication reconciliation services
  • Enhance patient adherence by discharge counseling
  • Provide patient counseling and subsequent follow-up for patients recruited under Integrated Care and Discharge Support for Elderly Patients (ICDS)
Drug Information Service
Clinical Pharmacists provide Telepharmacy service to patients
  • The Department of Pharmacy has implemented a Drug Information Service since August 1995, targeting doctors, nurses, other allied health professionals, patients and the public
  • Promote the safe, efficient and cost-effective use of medications for patients by the provision of updated and accurate drug information and advice
  • Provide updated drug information by the publication of drug bulletins
  • Manage all new drug applications and provide information support and advices on the drug applications
  • Periodically revise and update the Hospital Drug Formulary
Oncology Clinic
  • Provide pharmaceutical care to oncology outpatients by Oncology Pharmacists with aims to enhance patient side-effect management, improve therapeutic outcomes of the anti-cancer therapies and enhance self-management of patients
  • Target patients newly started with anti-cancer therapies who may have multiple chronic diseases
Pharmacist-led Ambulatory Clinics
  • The Pharmacist-led Ambulatory Clinics have been implemented in Queen Mary Hospital since 2009, serving patients ranging from those taking oral anticoagulants to patients with Diabetes Mellitus, Chronic Obstructive Lung Disease or Heart Failure, etc.
  • Provide personalised pharmaceutical care including International Normalized Ratio (INR) monitoring, dosage adjustment, therapeutic monitoring and patient counselling according to protocols
  • Use of video conferencing to provide Telepharmacy service for enhancing pharmaceutical care to patients if needed
Medication Delivery Service and Telepharmacy Service
  • Queen Mary Hospital Pharmacy is the first pilot site for Medication Delivery Service and Telepharmacy Service in Hong Kong. In March 2023, the Department of Pharmacy has implemented the Medication Delivery Service and Telepharmacy service for Telehealth clinics. In May 2023, these services have been rolled out to the Specialist Outpatient Clinics (SOPCs)
  • Provide seamless pharmaceutical care to patients with drug treatment and medication delivery option for patients after doctor consultation. With the Telepharmacy function of the Hospital Authority mobile application ‘HA Go’, Clinical Pharmacists review patient’s drug profile, assess patient’s adherence to drug therapy, and empower patients and caregivers with knowledge on disease management
Clinical Trials Pharmacy
  • Actively participate in the management of all investigational drugs with compliance to the Good Clinical Practice of The International Council for Harmonisation (ICH-GCP)
  • Collaborate with the Clinical Trials Centre, the University of Hong Kong to provide pharmaceutical services and advices for conducting clinical trials of new drugs in Hong Kong
  • Accredited by the National Medical Products Administration (NMPA) in 2012
